Ask a functional or integrative practitioner for "gut testing" and the phrase you will hear back is comprehensive stool analysis — often by its acronym, CDSA. It is not one product but a category of laboratory panel built to answer a deliberately broad question: what is going on in this gut, across digestion, inflammation and the microbial ecosystem, all at once? That breadth is the point — and the reason a comprehensive panel is not automatically the right purchase.

What a comprehensive stool analysis is

A comprehensive stool analysis is a single stool sample examined across multiple domains rather than for one suspect. A targeted test asks one question — is H. pylori present? A clinical PCR pathogen panel asks a slightly wider one: which acute infection explains this diarrhea? A comprehensive panel instead surveys the territory: whether you are digesting food properly, whether the gut lining is inflamed, whether the immune system of the gut is underpowered or overreacting, which pathogens, parasites and fungi are present, and whether the resident bacteria are in reasonable balance.

The term CDSA, for Comprehensive Digestive Stool Analysis, started life as the proper name of an early commercial panel and drifted into generic use. Today practitioners use it loosely for any broad stool panel, whatever technology it uses — and the technology is the variable worth understanding.

The classic CDSA versus modern molecular panels

The classic CDSA of the 1990s and 2000s was built from three tools. Culture: stool spread on growth plates to see which organisms grow. Microscopy: a technician examining slides for ova and parasites. Chemistry: assays for digestion markers such as fecal fats and enzymes. It was genuinely useful, and it had genuine blind spots — most gut bacteria are anaerobes that will not grow on a culture plate, and microscopy depends on an organism being present, intact and recognized on the slide. Parasites that shed intermittently classically required two or three separate samples.

Modern comprehensive panels such as the GI-MAP™ (Gastrointestinal Microbial Assay Plus) replace culture and microscopy with quantitative PCR: the lab detects the DNA of defined target organisms and reports how much of each was found. Organisms that never grew on plates are measurable, one sample suffices, and the output is a number rather than a technician's read. Fairness requires the reverse column too: an organism grown in culture can be tested directly against antibiotics, which no DNA panel replaces; a microscopist can spot something that is not on a panel's target list; and some laboratories' traditional panels report chemistry, such as short-chain fatty acids, that the GI-MAP™ does not. Neither generation of test is a diagnosis — both produce data for a practitioner to interpret.

What a comprehensive stool test measures, category by category

Digestion and absorption

Pancreatic elastase-1 reflects pancreatic enzyme output; low values are associated with poor digestion of fat and protein. Steatocrit measures fat actually left in the stool — the downstream evidence. Beta-glucuronidase is a bacterial enzyme practitioners consider in the context of estrogen and toxin recirculation.

Inflammation and immune markers

Calprotectin is the flagship: an inflammation marker used widely in conventional gastroenterology, where markedly elevated values prompt escalation to a GI physician (the NIH's MedlinePlus has a plain-language explainer). Secretory IgA reflects the gut's own antibody defense — low and high both carry meaning. Anti-gliadin IgA is a marker of immune reaction to gluten, not a celiac diagnosis. Occult blood screens for hidden bleeding, and zonulin, an optional add-on, is used to assess gut-barrier integrity.

Microbiology: pathogens, commensals and opportunists

Three layers. Bacterial and viral pathogens — the organisms nobody should host at high levels. H. pylori with its virulence factors, which help a practitioner judge how aggressive a strain may be. And the resident flora: normal commensals such as Akkermansia muciniphila and Faecalibacterium prausnitzii, where low counts matter as much as high ones, plus opportunistic bacteria that are tolerable at low levels and troublesome when overgrown — a pattern relevant to IBS and SIBO workups.

Parasitology and fungi

qPCR targets for parasites including Giardia, Cryptosporidium and Blastocystis, plus fungal organisms including Candida. Because detection is DNA-based, it does not depend on catching a live, intact organism under a microscope on the right day.

Antibiotic-resistance genes

The panel also reports genes associated with antibiotic resistance. This informs a prescriber's thinking about which drug families may fail; it does not replace culture-based susceptibility testing, and no stool test tells you on its own whether antibiotics are warranted at all.

Do you actually need a full stool analysis?

Not everyone does, and it would be easy money to pretend otherwise. A comprehensive panel earns its price when symptoms are chronic and unexplained — the bloating, irregularity and food reactions that have survived a conventional workup — and when a practitioner is ready to act on the results. It is also the right shape of test when several questions need answering at once — running targeted tests one by one gets slow and expensive.
